Healthcare and Social Security in China: What Every Expat Professional Needs to Know

I.  Social Insurance Participation Requirements

According to China's Social Insurance Law and the Provisional Measures on Social Insurance for Foreigners Working in China, foreign employees must enroll in the social insurance system if they meet all the following criteria:

✅ Hold a valid work visa (e.g., R visa, Z visa)
✅ Have signed a labor contract with a company registered in China
✅ Will be working in China for more than 6 months

Exemptions:

Foreign employees from countries that have bilateral social insurance exemption agreements with China (e.g., Germany, South Korea, Japan) may be exempt from certain types of insurance upon presenting a Certificate of Coverage.

Short-term business travelers (less than 6 months) are not required to enroll.

Outpatient: Annual deductible ¥500–¥1,500; reimbursement rate 50%–90% (higher for community hospitals)

Inpatient: Per-visit deductible ¥800–¥2,000; annual reimbursement cap up to ¥500,000 (varies by city)

2. Medical Services for Foreigners

Designated International-Friendly Hospitals (some support direct billing via medical insurance):

Beijing: Beijing United Family Hospital, Beijing International Medical Center

Shanghai: Ruijin Hospital International Division, Parkway Health

Guangzhou: Guangzhou International Travel Healthcare Center

VIP Clinics: English-speaking staff available; specialist appointments accepted

3. Medical Card Usage Steps

Receive your Social Security Card: Your HR will assist; issued within 15 working days

Bind Your Preferred Hospitals: Choose 1–3 designated hospitals (recommend one top-tier and one community hospital)

Treatment & Settlement:
▸ Register with your social security card
▸ Medical costs are directly covered by insurance
▸ For non-designated hospitals: pay upfront and apply for reimbursement later
